Description: Tuesday 1 hour personal care/ 1 hour domestic assistance MONRING SERVICES REQUIRED 9AM START Personal care 1 hour Showering Level of assistance: Supervision Equipment in use: Shower chair, there is a shower rail in the shower Mobility /Transfers: George uses his 4WF and walking stick Setting up area (clothes, towel etc): towel is on the rail in the bathroom. Undressing/ dressing: Only supervising, help when he needs. Assistance in the shower: No assisting just supervision. Getting dry: No assisting just supervision. Special requirements: Please ask George, also George likes to attend to his shower on time please arrive on time otherwise he will have a shower without supervision. After shower: George will usually have his breakfast and please clean the bathroom Domestic Assistance 1hour: aim for twice a week. -Location of cleaning products: Supplier to bring their own. -Additional tasks: None -Special instructions/ Preferences: Bring your own cleaning supplies, mops and vacuum. -Client participation: None -Tasks: • Change the linen and put new linen in bed • Hang washing out if available • Clean toilet (including toilet bowl) • Clean bathroom (including vanity, mirrors, surfaces and shower glass) • Clean Kitchen (bench, microwave) • Sweep and mop all floors Access to property: please note that the unit is located within Ingenia Lara facility. Upon entry, refer to the map of units. Parking: There is parking at the visit parking, or park at George's driveway. Alerts: Client is unsteady on their feet. Family present during service delivery: Client lives alone ON FRIDAY 1-hour personal care and 1 hour assisted transport/social ** Reason for transport (medical etc): George would like assistance with going to local shops (groceries/shopping), attending medical appointments completing personal errands (going to the pharmacy, getting a haircut). This may vary from week to week depending on her needs and activities. Pick up location: unit 203 40 Watt Street, Lara, VIC 3212 Destination: Local shops Equipment/ Mobility/ transfer: George is independent, but he has 4wheeled walking frame. Activity ideas/locations: George would like to access the local shops near his village. Communication: George communicate effectively, able to express his needs and engage in conversation independently. Toileting/continence support: Independent. Km's approved: (as long as it is within the 2 hours work timeframe)
