Position Overview:
As a Disability Support Worker/Aged or Disabled Carer at Morang Community Care and Services (MCCS), you will deliver high-quality, person-centred support to individuals living with disabilities across home and community settings. The role focuses on promoting participants’ well-being, independence, and quality of life by assisting with daily living activities, implementing support plans, and encouraging meaningful social and community participation.
You will work collaboratively with the care team, families, and allied health professionals to ensure supports are delivered safely, respectfully, and in accordance with NDIS Practice Standards and organisational policies. Strong emphasis is placed on documentation, accountability, and maintaining a safe and supportive environment for all participants.
Tasks & Responsibilities:
- Provide person-centred support to individuals with physical and/or intellectual disabilities in home and community settings.
- Assist with activities of daily living, including personal care, hygiene, grooming, dressing, and mobility support.
- Implement and monitor individual support plans to achieve participant goals and outcomes.
- Support participants with community access, social inclusion, and skill development activities.
- Maintain accurate and compliant documentation, including progress notes, incident reports, and medication records.
- Respond appropriately to incidents, behaviours of concern, and emergencies in line with policies and Behaviour Support Plans.
- Work collaboratively with families, carers, allied health professionals, and internal teams.
- Participate in team meetings, handovers, and ongoing training.
- Ensure compliance with NDIS Practice Standards, Work Health and Safety (WHS) requirements, and organisational policies.
Qualifications & Requirements:
- AQF Certificate III in Individual Support (Disability, Aged Care, or equivalent) or higher qualification.
- Minimum 1 year of relevant experience supporting individuals with disabilities.
- Demonstrated ability to deliver high-quality, person-centred support with a strong focus on participant dignity and choice.
- Proven ability to manage tasks effectively, including administrative duties and documentation, in a timely and accurate manner.
- Strong written, verbal, and interpersonal communication skills, with the ability to engage effectively with participants, families, and stakeholders.
- Ability to work collaboratively within a team environment, as well as independently under the direction of management.
- Sound understanding of NDIS practices, compliance, and documentation requirements (desirable).
- AUSLAN training (desirable but not essential).
- Knowledge or experience in diabetes management, including mealtime support and monitoring (desirable).
- Demonstrated commitment to ongoing learning, professional development, and continuous improvement.
