About the Role
This Community Services Coordinator is responsible for coordinating the delivery of community-based care services to clients, ensuring services are tailored to individual needs and promote independence, wellbeing, and social inclusion.
Key Responsibilities
- Coordinate and facilitate delivery of in-home and community-based care services in accordance with client needs and care plans
- Assess client requirements in consultation with clients, families, and care teams
- Match clients with appropriately skilled support workers
- Monitor service provision and respond to changes in client needs
- Liaise with clients, families, case managers, and external stakeholders
- Identify service gaps and implement solutions to improve service delivery and client wellbeing
- Support clients in accessing services that promote independence, health, and community participation
- Maintain accurate client records and ensure compliance with organisational policies and relevant standards
About You
- Diploma of Community Services or a related qualification
- At least 1 years’ experience in community services, aged care, disability services, or a similar field
- Demonstrated ability to assess client needs and coordinate appropriate services
- Strong communication and stakeholder engagement skills
- Ability to work collaboratively within a multidisciplinary environment
- Strong organisational and problem-solving skills
- Commitment to supporting vulnerable individuals and promoting independence
